Applicants must provide the following data:

1. Verification of income. Proof required includes, but is not limited to, at least one of the following:
   1. Last year's Income Tax Return
   2. Copy of all recent W-2s
   3. Current paycheck stubs
      
      
2. Verification of financial assistance, if applicable. Proof required includes, but is not limited to, at least one of the following:
   1. Letter from Welfare, Social Services, Disability, Unemployment or other appropriate agencies
   2. Copy of Social Security payments
   3. Food Stamp, AFDC, SSI, Old Age Pension, Medicaid or other verification
   4. Students, emancipation from parents. This fund is not set up to help the "traditional" student
   5. Financial Aid Paperwork
      
      
3. Address verification. Proof required includes, but is not limited to, at least one of the following:
   1. Recent utility bill
   2. Recent phone bill
   3. Lease agreement
      
      
4. Social Security Number
   
   
5. Current Identification Card or Driver's License
   

* Approval of applicant is based on gross income and the number of family members in the household. Eligibility and amount of discounted services are determined by the posted Income Guideline schedule.

* All applicants must agree to have their pets spayed or neutered as soon as health allows. Failure to comply will disqualify any further treatment of the pet.

* Payment is expected at the time of service unless a prior arrangement has been made. We reserve the right to discontinue services to clients who fail to pay their bill.
